Yoga

Overview

Yoga is a mind-body practice originating in ancient Indian philosophy that combines physical postures (asanas), breath control (pranayama), and meditation to promote flexibility, strength, autonomic balance, and psychological wellbeing. In cardiac rehabilitation, appropriately modified yoga is a Phase III-IV recommended modality for reducing sympathetic nervous system drive, improving heart rate variability, and supporting long-term stress management.

Not all yoga is safe for cardiac patients. Specific categories of postures and breathwork are contraindicated due to their hemodynamic effects.

  • Phase applicability: Phase III-IV only (not appropriate in Phase I-II acute setting)

Contraindicated Categories for Cardiac Patients

Inversions and Semi-Inversions

Headstand (Sirsasana), Shoulderstand (Sarvangasana), Plow Pose (Halasana), extended Downward-Facing Dog. Risk: Gravity-induced venous return surge causes acute volume overload; carotid sinus compression triggers bradycardia/syncope.

Deep Backbends

Wheel Pose (Chakrasana), Camel Pose (Ustrasana), Bow Pose (Dhanurasana). Risk: Intense chest cavity pressure, elevated heart rate, dizziness, shortness of breath.

Deep Torso Twists

Seated Spinal Twist (Ardha Matsyendrasana), Revolved Triangle (Parivrtta Trikonasana). Risk: Abdominal vessel compression causes erratic blood pressure shifts on pose release.

Isometric Overhead Arm Holds

Warrior I with arms raised, Tree Pose held overhead. Risk: Sustained isometric arm elevation increases systemic vascular resistance and afterload.

Forceful Breathwork (Pranayama)

Kapalbhati, Bhastrika, any Kumbhaka (breath retention). Risk: Mimics Valsalva maneuver; power/hot yoga causes dehydration and electrolyte imbalances leading to arrhythmia risk.
